Referrals

We accept referrals either from staff in the division of youth psychiatry or from community physicians.

To make a referral,

1. Physicians should contact the program coordinator at 416-480-6100 ext. 62506 to discuss potential referrals.
2. Referring physicians should review the ‘Information for clinicians' page to ensure appropriateness and then complete a physician referral form
3. Patients should complete a self-report form 
4. Fax both completed forms to the Fresh Start Program Coordinator at 416-480-6818; however, referrals will not be considered until after the referring physician has contacted the program coordinator to discuss the referral.

For the duration of a student's enrollment in the Fresh Start program, the program's psychiatrist will provide psychiatric care.
