Quality Improvement Plan

In June 2010, the Ministry of Health and Long-Term Care in Ontario implemented the Excellent Care for All Act. This Act requires that all hospitals in Ontario put annual quality improvement plans in place and make these available to the public.

Sunnybrook has finalized its third annual Quality Improvement Plan (QIP) for 2013 / 2014. The plan outlines the hospital's priorities for quality improvement, and also provides accountability for reaching the highest possible quality and value for health care in Ontario.

The Quality Improvement Plan is made up of two parts. You can download the PDF for each by clicking on the icons below:

 Part AA brief overview of the hospital's quality strategic goals and the main areas of focus in the Quality Improvement Plan, an overview of performance-based compensation for our executives and an accountability sign-off.

 Part B: A spreadsheet describing our improvement targets and initiatives. The spreadsheet includes a core set of indicators that many hospitals across the province are working on and other indicators that Sunnybrook has identified as a priority.

 Progress Report on Quality Improvement Plan 12/13: A report on last year's plan which demonstrates the significant progress we have made to date on our high priority indicators in the 2012/13 Quality Improvement Plan.

The Quality Improvement Plan is only one of the many ways we are working to improve our patients' experiences.
