Adverse drug reacting reporting

If you see or treat a patient with Stevens-Johnson syndrome (SJS) or toxic epidermal necrolysis (TEN), we ask that you submit an adverse drug reaction report .

We are a research group examining the drugs associated with TEN/SJS and their treatment. We ask that you print off the adverse drug reaction report form available from the Canadian Adverse Drug Reaction Monitoring Program (CADRMP) or the PDF files available below. Please complete and send to our toll-free fax number: 1-866-520-9042. We will enter the information received in a registry and submit to the national reporting program.
