Aging Well Series: Advance Care Planning

The Sunnybrook Academic Family Health Team (SAFHT) will be offering our Aging Well series virtually via Zoom. The Aging Well series is designed to provide evidence and group-based education for individuals on healthy aging – how to keep well and be well!

Did you know that…

  • 8 in 10 Canadians have given end-of-life care some thought, but less than 1 in 5 has an advance care plan (ACP).
  • You do not need a lawyer to start Advance Care Planning and can change at any time.

Advance Care Planning (ACP) allows you and your loved ones to reflect on your own values and wishes and to let others know what kind of health and personal care you would want if you became incapable of consenting to or refusing treatment. ACP can be a very difficult topic to initiate with your loved ones. However, talking about your values and goals will give you and your loved one’s peace of mind and may be an essential component of your healthcare journey.

This two-part online workshop will help support you, your family, and your friends to begin your advance care planning journey.

  • Presented by: Occupational Therapist, Care Navigator, Social Worker, Registered Nurse
