End of life and care after death documentation

In order to provide the best quality care for patients and their families coping with challenging, advanced or life-threatening illness, care teams need to discuss and document how people wish to be cared for at the end of their lives and after death.

We are interested in speaking with patients and families who can provide feedback on our ‘End of Life’ and ‘Care After Death’ Documentation.

  • A follow-up phone call will be organized with our team to discuss your thoughts (approx. 30 mins).
