Help enhance person-centred care

We are seeking patients and caregivers to join Sunnybrook’s Person-Centred Care Committee. The Committee is responsible for designing, developing, implementing, monitoring, and providing oversight of the organization’s approach to patient care.

Term:

This is a two-year commitment.

Meetings:

The committee meets on the first Monday of each month, excluding July and August. Partners also have the option to attend via teleconference.

Location:

2075 Bayview Avenue, Toronto, ON M4N M35. The cost of your parking or TTC fare will be reimbursed.

Key responsibilities:

  • Advance person-centred care at Sunnybrook through the integration of the three Core Principles and Three Vital Behaviours of Seeking and Embedding the Voice of the Patient
  • Advise on objectives and champion activities that advance the full and consistent integration of person-centred care into everyday clinical activities.
  • Provide leadership across Sunnybrook with respect to current evidence, innovative ideas, implementation strategies, sustainability and spread of person-centred care
  • Monitor key inputs as process and outcome indicators (e.g. organization’s patient satisfaction/ experience survey data, Office of the Patient Experience data, "Conversations with Patients" data, verbatim comments from mailed standardized post-discharge patient questionnaires, and other sources of feedback), in order to evaluate the effectiveness of the approach
  • Act on and implement changes based on results and information from key inputs, as necessary
  • Routinely engage in broader consultation with staff within the areas/ professions/ teams represented by the membership, and with patients and family members, in order to receive feedback about how to advance the corporate approach and sustain change over time
  • Facilitate and champion dissemination of information exchange within the areas/ professions/ teams
  • Advocate for patient and family member partners’ participation in Sunnybrook’s committees
  • Respond to ongoing recommendations with respect to prioritization, alignment and timing of person-centred care activities, in coordination of other organizational activities
  • Advise the Accreditation Team on key patient engagement initiatives to promote with frontline care providers, in support of meeting the Accreditation Canada standards for client- and family-centred care
  • Monitor developments and innovations in person-centred care outside of the hospital and champion the relevant ones to be integrated into Sunnybrook’s approach
  • Facilitate and champion the spread of person-centred care within our community and contribute to system leadership in this area
  • Provide regular reports of progress to the Interprofessional Quality Committee
  • Advocate for supports required to successfully integrate person-centred care
