Sunnybrook Heart Function and Rapid Cardiology Assessment Clinic

The Heart Function and Rapid Cardiology Assessment clinic (HF&RCAC) at Sunnybrook Health Sciences Centre is focused on providing timely assessment of patients with congestive heart failure, chest pain, and arrhythmia who do not require urgent admission. Our primary mandate is to reduce hospitalization and re-readmission by facilitating access to cardiologist.

One of the rapidly expanding areas in the HF&RCAC is centered around the management of patients who have heart failure. Despite advances in medical therapy, heart failure currently affects over 600,000 Canadians. According to Heart and Stroke Foundation statistics, about 50,000 Canadians are added to that list every year. Heart failure has now come to be recognized as a growing epidemic that places tremendous burden on patients, families and the health care system.

The primary objective of the Sunnybrook Heart Function Clinic is to provide leading care to patients with all forms of heart failure, with an emphasis on patient engagement and development of self-care capabilities, through the use of innovative strategies focused on educating patients, families, and their other health care providers, so that ultimately quality of life will be improved and hospitalizations can be prevented.

Benefits of the Sunnybrook Heart Function Clinic:

  • Provide comprehensive patient-centered care for heart failure patients not requiring urgent admission
  • Individual consultations with a Nurse Practitioner or Registered Nurse and a heart failure cardiologist.
  • Patient-centred education about heart failure
  • Optimization of heart failure medical therapy and assessment for advanced therapies (i.e. candidacy for either cardiac transplant or left ventricular assist devices)
  • Provide information and/or counseling about lifestyle changes
  • Referral to outpatient cardiac rehabilitation as needed.

Referral Criteria for the Heart Function and Rapid Cardiology Assessment Clinic:

  • Congestive Heart Failure not requiring urgent admission: facilitate assessment within 2 weeks of discharge from ER or hospital ward
  • Low risk chest pain: consideration of coronary etiology
  • Arrhythmias requiring urgent follow-up (mainly atrial fibrillation patients): SVT patients in whom ablation is being considered should be referred to the EP clinic unless there is a need for urgent clinical review within 48 hours.
  • As an option to an ER assessment when indicated for family physicians within Sunnybrook Hospital and its catchment area

PLEASE NOTE THE CLINIC IS NOT AVAILABLE TO REVIEW NON-URGENT CARDIOLOGY PATIENTS, OR ARRANGE A TRANSFER OF CARE FROM ONE CARDIOLOGIST TO ANOTHER.
