What happens when my loved one is ready to leave the ICU?

As soon as a patient no longer needs ICU care, the team will put together a transfer plan.

Where the patient is sent next depends on a number of factors including the patient’s medical condition and care needs.

There are several possible transfer pathways:

There are many rehabilitation hospitals across the Greater Toronto Area. A social worker is usually the team member responsible for making the arrangement to send the patient to another hospital. So, the social worker should be able to provide you with more detailed information related to your loved one’s discharge plan.
