Patient Care Manager II - Ambulatory Care Services - Hurvitz Brain Sciences Program - Regular Full Time (220697)

We currently have a Regular Full-time opportunity for a Patient Care Manager II in the Ambulatory Care Services - Hurvitz Brain Sciences Program at our Bayview Campus.

Hours of Work:

Monday to Friday Days (some flexibility required)

Summary of Duties

The Patient Care Manager (PCM) is responsible for Ambulatory Care Services in the Hurvitz Brain Sciences Program, including Mental Health Ambulatory Clinics, Electroconvulsive Therapy (ECT), Sunnybrook Program of Assertive Community Treatment (SUNPACT), Community Psychiatric Services for the Elderly (CPSE), and Medication Clinic. This includes responsibility for leadership and operations of the services, professional practice, and the quality of care. The PCM leads and collaborates with the interprofessional team to deliver innovative services, achieve operational efficiency, manage budgets and resources, develop a high-performing team and a positive working environment, and person-centred care. The PCM works with the Program Leadership team to achieve program goals. The PCM is also accountable for ensuring that care requirements are met, and that patient care issues and concerns identified by patients and their families are addressed.

Qualifications/Skills

  • A Master’s Degree in Clinical/Health Sciences or Management preferred, Bachelor’s Degree required.
  • Current certificate of competence from Professional College.
  • Minimum of 3 years recent Management experience in healthcare setting.
  • Experience in managing ambulatory/community mental health services strongly preferred.
  • Demonstrated knowledge and experience with the Mental Health System and mental health patient populations strongly preferred.
  • Strong demonstrated leadership, communication, coaching, team building and decision–making skills.
  • Experience with change management and innovation.
  • Demonstrated interpersonal and collaboration skills.
  • Knowledge of legislation affecting human resources (i.e. human rights, employment standards, Occupational Health & Safety).
  • Familiarity with collective agreements, scheduling, and budgets.
  • Computer literate.
