Social Worker - Tory Trauma Program - Critical Care Unit & D4 Intensive Care Unit - Regular Full Time (221260)

We currently have a Regular Full-Time opportunity for a Social Worker in the Tory Trauma Program - Critical Care Unit & D4 Intensive Care Unit at our Bayview campus.

Hours of Work:

Weekdays and Weekends as per Rotation: Days; 8 hours; As per schedule

Summary of Duties

To provide a full range of social work services to patients and their families admitted to Critical Care and D4ICU (for non-trauma service patients) within the Trauma program. This includes psycho-social assessments, counselling to patients and families, end of life care, Advance Care planning and goals of care conversations, transition planning, crisis intervention, linkage to community resources, team consultation.

Qualifications/Skills

  • Masters of Social Work Degree Required
  • Member of the Ontario College of Social Workers and Social Service Workers in Good Standing
  • Minimum two (2) years of experience working in a Hospital setting
  • Experience in Trauma or critical care preferred
  • Exceptional communication skills required
  • Knowledge of the HCCA and SDM act
  • In-depth understanding of the Sunnybrook Policies that impact transitional planning
  • Strong negotiation and conflict resolution skills
  • Demonstrated skills in Inter-professional Collaborative Practice & Education
  • Ability to balance collaborative practice with self-direction
  • Ability to manage multiple tasks and set priorities in a fast-paced environment
  • Commitment to student education
  • Experience and commitment to quality improvement initiatives
  • Demonstrated knowledge of organizational priorities and social work scope of practice priorities
