Introducing your North Toronto Sub-Region winter crew

As the winter season approaches, we know that there are increases in illness and injury, especially due to the spread of the flu. This results in increased demand on primary care clinics and emergency departments. As an integral partner of primary care physicians in the region, the Toronto North Interprofessional Primary Care Team (IPCT), in partnership with PCCC and SPRINT Senior Care, is taking a leadership role to implement a surge plan to support its family physicians in three ways.

1. Vaccination clinics and health promotion

During the months of Nov. and Dec. – IPCT will provide health promotion information and run vaccination clinics in 11 hot spot seniors’ buildings throughout the region. Please look out for a fax from us to let you know that your client has been vaccinated.

2. Increased after-hours and weekend support

During the period from Dec. 15th – Feb. 28th, IPCT will offer designate some evening and weekend hours to provide virtual access to NP/RN services. A schedule will be sent closer to the start date.

3. The winter crew

A collaboration between IPCT and SPRINT Senior Care. The Winter Crew is a team supporting clients for a short period of 48 hours to 5 days during the acute state of their illness/injury to help with:

  • IPC: Home assessments to ensure safety and comfort (RN/NP/OT/PT/RD); Pharmacist services to support any change in medication through education and/or do a medication review.
  • SPRINT Senior Care: Meal delivery ensures patients maintain good nutrition if they cannot cook and do not have sufficient social supports; Rides to appointments and any other home support i.e. PSW support, safety checks, etc.
