Telehomecare: a new way to treat individuals with CHF and COPD

Telehomecare links patients with chronic conditions to registered nurses who provide remote monitoring and health coaching sessions. Patients become partners in their care as they learn to manage their health at home.

Telehomecare will increase support and self-management of patients living with chronic conditions to ensure patients receive the right care, at the right time, in the right place.

Telehomecare may be the right option for your patient if they:

  • Have an established diagnosis of congestive heart failure (CHF) or chronic obstructive pulmonary disease (COPD)
  • Have had repeated emergency department visits and/or hospital readmissions
  • Are capable of using simple in-home monitoring equipment  

Completed referral forms can be faxed to 416-217-1439 or emailed to telehomecare@toronto.ccac-ont.ca.
