Making an appointment

Appointments are made by referral from your doctor.

Referring a patient

To arrange an appointment for a patient, please fax your referral directly to the referral doctor. For information about areas of interest and specialty, please consult the individual physician links. Referrals addressed to GI clinic will not be accepted. 

Be sure to include all patient demographics and reason for referral. Labs and images are not required for an appointment.
