For every visit, you must bring:Schedule icon.

  • Your Ontario health card (green) and your Sunnybrook hospital card (red).
  • A list of medications (drugs) you are taking like prescription and non-prescription (over-the-counter) medicines, vitamins, herbal products, alternative therapies, etc.
  • Equipment you may need (ostomy bags, oxygentanks, etc.)
  • Money/credit card or drug insurance information (group and policy numbers for private insurance, Trillium, Ontario Works, Ontario Disability Support Program, etc.)

You are responsible for the cost of drugs prescribed that are not covered by Ontario Health Insurance. If you do not have drug coverage, please contact our drug reimbursement specialist (416-480-4623).

We suggest you also bring:

  • Food/snacks and drinks as you may be at the centre for most of the day
  • Money/credit card to pay for parking. Visit our parking information page for more information on parking rates.
  • A list of questions to ask your health care team. Writing down your questions/concerns before coming to your appointment will help you remember to ask about things that are important to you. Some questions you may want to ask are:
    • What is the purpose of my appointment?
    • I have symptoms. Can we talk about them?
    • What happens next?
