Referrals

We accept referrals from either staff in the Division of Youth Psychiatry or from community physicians and/or psychiatrists.

Referrals should come from the patient’s psychiatrist or primary care physician.

Referrals should be accompanied by copies of all previous (i) psychiatric assessments, (ii) psychoeducational testing reports, and/or (iii) any other reports relevant to the patient’s mental health.

To make a referral,

1. Physicians should contact the program coordinator at 416-480-6100 ext. 62506 to discuss potential referrals.
2. Referring physicians should review the ‘Information for clinicians' page to ensure appropriateness and then complete a physician referral form
3. Patients should complete a self-report form
4. Fax both completed forms to the Fresh Start Program Coordinator at 416-480-6818; however, referrals will not be considered until after the referring physician has contacted the program coordinator to discuss the referral.

While students are attending the Fresh Start program, the program's psychiatrist and mental health treatment team will take over all aspects of the student’s mental health care.
