Referral information
Thank you for your interest in referring your patient to the Department of Dental and Maxillofacial Sciences, Sunnybrook Health Sciences Centre.
In order to ensure that we can offer excellent care for patients while following the guidance of the Ministry of Heath, our regulatory bodies and the Hospital, a completed referral form (see below) is required.
All referrals will be screened for suitability based on patient’s medical history and complexity of care. Once screened, we will communicate details and requirements with the patients and referring offices.
Please note there are currently long wait times for patients to be seen in the Oral Medicine and Oral Pathology Clinic.
