Geriatric Medicine

We offer a full spectrum of patient care for seniors, from multidisciplinary assessments to outreach & in-home treatments. We have an active day hospital, a falls program and provide consultations to all areas of the hospital, as well as the Veterans & Community Program.

Making an appointment

Most of our services require a referral from your health care provider. Please contact your physician for a referral to our clinics.

Referring a patient

Call the Geriatric Referral Office at 416-480-6100 ext. 66888 and fax all related documentation and lab work to 416-480-4778.

Residential Program Referrals
Initial application to the Sunnybrook Veterans Centre (L and K Wings) is made through Veterans Affairs Canada (VAC).
