Sessions offered

By meeting with the SUNDEC team in individual appointments or group sessions you will be able to:

  • Engage in self management behaviours to better manage your diabetes.
  • Understand the role of diabetes related medications.
  • Learn to incorporate healthy food choices and healthy portions.
  • Discover the benefits of adding exercise to your daily routine.

  • Healthy habits for pre-diabetes
  • Living healthy with type 2 diabetes
  • Aging well with diabetes (70 years +)
  • Keeping current with type 1 diabetes
  • Craving Change™: A How-to Guide for Changing Your Relationship with Food
  • Connections: Keeping You Healthy With Chronic Conditions (a self-management program)

Individual sessions

  • One-on-one appointment with a diabetes nurse educator and/or dietitian
  • We work with you to personalize a diabetes care plan that fit into your life

Self-referral »

Patients may self-refer by calling 416-480-4805 between 8 a.m. – 4 p.m. Patients may also leave a voicemail, and a staff member will return the call.
