Low Back Pain Rapid Access Clinic: For primary care providers
Referrals to the Low Back Pain Rapid Access Clinic are only accepted from primary care providers that have Rapid Access Clinic (RAC) Low Back Pain referral privileges.
Primary care providers must be enrolled in RAC Low Back Pain in order to refer patients. Once enrolled, they will receive RAC Low Back Pain referral privileges and will be able to refer patients to receive timely low back pain assessments (within four weeks), education about their condition, evidence-based self-management plans, and additional assessments and treatments where required.
If you are a physician or nurse practitioner interested in enrolling in RAC Low Back Pain:
- Register for the online training module.
- Complete the online training module. This 15-minute training module is a mandatory part of the primary care provider enrollment process and is required to obtain referral privileges.
- Receive the referral form within two to three business days via email. Once you have received the referral form you may begin referring patients into RAC Low Back Pain.
Rapid Access Clinics (RAC) for Low Back Pain are being rolled out across the Toronto Central Local Health Integration Network (LHIN) to help improve the quality, access and appropriateness of care for people with low back pain.
