Nurse-Led Outreach Team

The Nurse-Led Outreach Team (NLOT) is a team of registered nurses at Sunnybrook that provide skilled, time-sensitive services to senior residents in the comfort of their own North Toronto Long-Term Care (LTC) Home.

Our services

  • Weekly in-home assessments and patient monitoring
  • Advanced nursing care, including: medication review, G tube replacement/management, difficult foley insertion, CVAD maintenance and care, IV administration, clinical assessments
  • Facilitation of urgent diagnostic imaging (Xray, CT, ultrasound) and other outpatient services (e.g. specialist referrals)
  • Consultations in-person, by telephone or email
  • Joint care planning with the primary physician and healthcare team
  • Resources and education for complex nursing skills

The Nurse-Led Outreach Team serves the following Long-Term Care Homes in North Toronto:

  • Sunnybrook Veterans Program
  • Isabel and Arthur Meighen Manor
  • Suomi-Koti Toronto Nursing Home
  • Arirang Korean Long-Term Care Home
  • Apotex Centre, Jewish Home for the Aged (Baycrest)
