Leverage system and peer-level quality markers to drive performance improvement

Develop and implement a comprehensive quality improvement approach focused on surpassing system and peer-level quality of care benchmarks.
  • Develop an inventory of system- and peer-level quality markers being used by the Programs.
    (Examples include National Surgical Quality Improvement Program (NSQIP); Trauma Quality Improvement Program (TQIP); Greater Toronto Area Obstetrical Dashboard (OBSGTA); Society of Thoracic Surgeons for adult cardiac surgery (STS); Intensive Care Observational Registry (iCORE); Burn Quality Improvement Program of the American Burn Association (BQI); etc.)
  • Ensure that targets and timelines have been set by the Programs to achieve and surpass benchmarks.
  • Establish a mechanism to prioritize and monitor system-level performance improvement in relation to quality markers.
  • Rationalize governance of Quality Improvement and Quality Improvement databases at Sunnybrook and streamline Quality Improvement investments.
  • Continue to monitor achievement towards targets and address performance and process issues.

Reduce practice variations

Optimize performance and reduce practice variations in key Quality Based Procedures (QBP) components to improve outcomes.
  • Review the elements of each QBP to identify prominent practice variations.
  • Develop targeted variation-reduction workplans with timelines, and begin to implement.
  • Review progress made to reduce practice variations and address outstanding issues.
  • Review progress made to reduce practice variations, address any final issues, and complete the workplans.
Continue developing and implementing Standard Order Sets and integrate into SunnyCare. 
  • Achieve Medical Advisory Committee (MAC)-approved goals for implementing QBP and non-QBP Standard Order Sets.
  • Set goals to align Standard Order Sets with Choosing Wisely Canada evidence-based guidelines, establish timelines and begin to implement.
  • Continue to align Standard Order Sets with Choosing Wisely Canada evidence-based guidelines, according to set timelines.
  • Integrate Standard Order Sets developed in 16/17 into SunnyCare.

Accelerate the uptake of Precision Medicine toward achieving measureable improvements in patient care.

  • Define the scope of “Precision Medicine” being considered.
  • Establish a Working Group (made up of Cognate Program Research Directors, Platform Coordinators) to:
    1. identify areas of clinical/ research intersections within Sunnybrook’s strategic priorities;
    2. develop guiding criteria to select intersections of focus for the next three years;
    3. establish indicators and metrics of successful impact on quality of care in the focus areas.
  • Implement efforts to advance intersections in the selected focus areas, and collect data on indicators and metrics.
  • Monitor and record progress made impacting on quality of care.
  • Fine tune the indicators and metrics, as required.
  • Continue implementation and monitoring and recording efforts.
