Partners

Sunnybrook defines its partnering element in three ways. One is the partnership between staff and patients to improve care for patients, and enable them to provide their expertise, experience and advice to make care better. The second is the partnership between Sunnybrook and other health-care providers so that patients get a great care experience both at and when they leave Sunnybrook. The third meaning is the partnership between Sunnybrook and other organizations to improve broader systems of quality.

  • Develop an inventory of current partnership arrangements that emphasize improving quality of care.
  • Assess the impact and effectiveness of partnerships on improvements in quality.
  • Continue implementing areas for improvement and updating the inventory including assessments of impact and effectiveness.

Develop integrated care delivery and funding models that specifically focus on improving quality.

  • Identify two partners with which to develop integrated care delivery and funding models to better serve two defined patient populations, and begin collaborative planning discussions with each partner.
  • Work with one partner to build and implement the model for one of the defined patient populations.
  • Work with the second partner to build and implement the model for the second defined patient population.
