Recognition
The SUNDEC/SAFHT (Sunnybrook Diabetes Education Program) has been granted recognition for successfully meeting the Standards of Diabetes Education in Canada. The review findings indicate that that “this program delivers superior diabetes education services for people in the community it serves.”
To attain recognition status, a Diabetes Education Centre must demonstrate achievement of a minimum of 80% of the standards within each of the Outcome, Process and Structure categories. SUNDEC achieved the maximum of 100% of the standards in each category. The review team described the Sunnybrook submission as “excellent with the appropriate level of detail outlining an adaptive, innovative program.”
The review committee highlighted several areas of commendation, including:
- Excellent staffing resources with historical record and high level of competence
- Working towards EMR.
- Strong focus on staff / client education and relationships.
- Impressed with the innovative programs of this centre.
- Client centred care is evident,
- Evaluations are ongoing,
- Partnerships and quality are considered in process work.
- Excellent coverage of essential quality program indicators i.e. triage, marketing, community partnerships, chart audits, and focus on self-management.
Recognition status is granted for a five-year period.
