Self-management tools

We have included a few forms for you to print and use for self-management.  You can fill in and bring to your appointment with your diabetes team.
 
This information helps us help you manage your diabetes.
 
My Blood Glucose Log Sheet
We recommend if you are monitoring your blood glucose to write down your blood sugar readings. If your doctor has already recommended testing times, follow those directions. If not, we recommend 1-2 tests a day, alternating the times.
 
For example: if you test your blood sugar once daily you may test fasting one morning and the next day test before dinner, then test one day before lunch. It may also be helpful to sometimes test your blood sugar 2 hours after you have eaten.

My Food Diary
In order to assess your eating habits, it is most helpful to fill out a 3 day food record, including what you eat, how much you eat and when you ate it. You may need to measure out some portions with measuring cups to see more accurate portion sizes.
