Subsequent Pregnancy Program

We care for women who are experiencing a pregnancy after a previous late-pregnancy loss or death of their baby during pregnancy (greater than 15 weeks), or women who have had a baby die shortly after birth (neonatal death). Women have access to a team of health care professionals who provide expert clinical care with sensitivity and continuity.

Care options are individualized to each woman and her family, with the goal of providing seamless and holistic care throughout the childbearing year. We offer preconception counselling and care during pregnancy and the initial postpartum period. The clinic has relationships with many community supports, including PAIL Network and Ontario’s Healthy Babies Healthy Children Program.

Some of the care options include:

  • Timely access to care providers between scheduled visits
  • Assistance with preparing your birth plan
  • Peer support through community partner (telephone and in-person support group)
  • Drop-in fetal heart rate checks
  • Prenatal Breastfeeding Clinic consultation
  • Prenatal referral to Public Health: Healthy Babies, Healthy Children Program
  • Individualized matching of community and pregnancy loss resources
  • Individualized education throughout pregnancy and the postpartum period
  • Private tours of Inpatient Units (Labour and Delivery, Maternal Newborn Unit, High Risk Obstetrics Unit)
  • Individualized prenatal visit schedule
  • Program-wide identification
  • Individualized postpartum follow-up (for approximately six weeks)

Women are seen in the Subsequent Pregnancy Program if they are interested in, or receiving ongoing pregnancy care, at Sunnybrook. We accept referrals from inside and outside of Sunnybrook, including from family physicians, obstetricians, psychiatrists, nurses, social workers, and midwives. If you are interested, please have your care provider fax a referral, and note "Subsequent Pregnancy Program."
