Nurse Clinician –Geriatric Outreach Team (GORT) - Specialized Geriatric Services - Temporary Full-Time  (221758)

We currently have a Temporary Full-Time (Until approx. January 2023) opportunity for a Nurse Clinician –Geriatric Outreach Team (GORT) in the Specialized Geriatric Services at our Bayview Campus.

Hours of Work:

Weekdays: Days; 8hrs; As per schedule

Summary of Duties:

As a member of Specialized Geriatric Services the Nurse Clinician works in community outreach. This position provides assessment, consultation and community linkage for frail seniors in their homes. In collaboration with other team members, the Nurse Clinician will:

  • Administer a Comprehensive Geriatric Assessment (CGA), plan, implement and evaluate care of elderly persons with complex geriatric health problems in the home
  • Formulate a coordinated care plan with older adults that is designed to address conditions identified in the CGA and to meet the goals of the older adult.
  • Provide follow up to monitor implementation of recommendations and adjust plans as necessary
  • Participates in team meetings to review client care
  • Provide education directly to older adults, families and caregivers
  • Collect data indicators and monitor performance


  • Bachelor’s degree in Nursing required. Master’s degree in Nursing preferred.
  • Current College of Nurses of Ontario Certificate of Competence; current B.C.L.S.
  • Additional training and/or experience in Gerontology is required
  • Experience working in community health care
  • Working knowledge of community resources, LHIN, RGP, and specialized geriatric programs
  • Excellent oral and written communication skills, and computer skills
  • Demonstrated ability to function on an interdisciplinary team
  • Knowledge and skills in functional assessment and the assessment of physical and mental illness in the elderly
  • Excellent organizational skills, problem solving and management skills
  • Ability to adapt to change, be flexible in an ever-changing environment
  • Valid driver’s licence and reliable vehicle for travel within the community required
