Toronto Central Local Health Integration Survey

Sunnybrook has been asked by the Toronto Central Local Health Integration Network, an agency of Ontario’s Ministry of Health and Long Term Care, to survey patients at registration regarding certain personal information related to preferred language, length of time in Canada, race or ethnicity, religion and so on. Your consent is required for Sunnybrook to collect this information using this survey. You may refuse to answer any or all questions in the survey. By completing any or all of the questions in this survey, you consent to Sunnybrook collecting this information from you and for its submission to the Toronto Central Local Health Integration Network in a form that does not identify you personally.

Why are you asking these eight questions? 

We have been asked by the Toronto Central Local Health Integration Network to ask these eight questions. The Toronto Central Local Health Integration Network carefully reviewed information about quality and equitable (fair and equal) care when deciding the questions.

Is it legal to ask these questions?

Yes, the Ontario Human Rights Commission encourages organizations to collect and use demographic information to keep track of outcomes and promote equity. Health equity refers to the study of differences in the quality of health and healthcare across different populations.

Who can see the demographic information?

Sunnybrook takes your privacy very seriously. The information collected will be stored in a database and not available to members of your health care team. The information will be submitted to the Toronto Central Local Health Integration Network, not including personal health information that will identify you as our patient.

How will you use this information?

The Toronto Central Local Health Integration Network will study whether factors such as language, disability, gender and so on are linked to health outcomes.

What if there are questions that I don’t want to answer?

You have the option of completing the survey or choosing “prefer not to answer” to any or all questions. This will not impact the care you receive here.
