Ontario sets official date to terminate the use of red and white health cards
Effective July 1, 2020, all red and white health cards will no longer be accepted for OHIP-insured services. Red and white card holders will have to update to a photo health card. The conversion to a photo health card is mandatory.
In January 2020, notices were sent to red and white card holders advising them of the need to obtain a photo health card. Patients who do not renew their red and white health cards prior to July 1, 2020 will be responsible for payment at the time care is provided at Sunnybrook since their cards will no longer be valid.
If you or a family member have a red and white card, we urge you to take the necessary steps to obtain a new photo health card and ensure continuous OHIP coverage.
- Complete the Health Card Re-Registration Form
- Collect the Required Proof of Identification
- Visit Service Ontario with the completed Health Card Re-Registration form, along with the required proof of identification
