3 East/Special Care Interprofessional Team, Holland Bone & Joint Program

The 3 East inter-professional team cares for patients on a 28-bed acute surgical postoperative unit that includes a Special Care (step-down) unit. The 3 East team’s commitment to excellence is evident in every aspect of care, from the moment a patient arrives to the unit, through their recovery, to safe same-day discharge or supported hospital stay.
By embracing innovation and working together, the team helped increase joint replacement surgical volumes from 2,200 to 3,500 per year, giving hundreds more patients access to life-changing joint replacements. Their success with the TRAC program implementation has improved wait times across the community while maintaining high standards of safety and care and strengthening Sunnybrook’s role as a regional leader in orthopedic care.
The key to this success lies in the spirit of “all hands in” teamwork to ensure safe, efficient, and compassionate care. Every patient is cared for with dignity, every staff voice is valued, and every role is recognized.
ALS Clinic Team, Barzakay Brain Health Clinic, Hurvitz Brain Sciences Program

The Sunnybrook ALS Clinic Team delivers compassionate, multidisciplinary care to patients and families navigating the challenges of amyotrophic lateral sclerosis (ALS). Their work directly advances Sunnybrook's Strategic Direction 2: Integrated Care, by leading system transformation with partners to expand and integrate primary and community care.
Through a collaborative, interdisciplinary model, the team provides a seamless experience that integrates neurology, respiratory therapy, occupational therapy, nursing, speech-language pathology, nutrition and social work with community palliative care, primary care, OHAH and the ALS Society of Canada.
The ALS Clinic Team has transformed the experience of thousands of patients and families living with ALS in Ontario by coordinating care across disciplines and care settings, they have:
- Reduced avoidable emergency visits and hospitalizations
- Improved quality of life through earlier symptom management and proactive interventions
- Enabled smoother transitions to community and palliative supports
Central Booking Department, St. John’s Rehab Program

The Central Booking Department at St. John's Rehab (CBD-SJR) plays a critical role in ensuring seamless patient flow from Sunnybrook proper and other regional partners into our specialized rehab program. This team works closely with social workers, transitional care coordinators, physicians and inter-professional clinical staff to support timely transfers, directly alleviating acute care occupancy pressures across the healthcare system.
By facilitating timely transfers of patients to rehab, they help relieve acute care occupancy pressures, ensuring beds are available for patients in need of urgent and specialized care. This contributes directly to system efficiency, improved patient flow and better outcomes for patients and families.
The team collaborates with a wide range of partners including clinical staff, support services, leadership and community partners ensuring that diverse voices and perspectives inform decision-making. By creating an environment where all stakeholders are heard, the team fosters trust and inclusion both within the organization and externally.
CT Clinical Team, Medical Imaging, Precision Diagnostics and Therapeutics Program

The CT Clinical team contributes to the patient experience by providing CT scans that assist with diagnosis, treatment and ongoing monitoring. A CT scan often is one of the first interventions a patient may have prior to determining a care pathway, so it is critical this procedure be completed as soon as possible.
The team implemented a plan to improve patient flow from the Emergency Department and took the initiative to create capacity. An ED dashboard was created that monitors the CT turnaround time from order to scan for ED patients. The team has consistently met or exceeded the established targets, making a huge impact on patient care.
The entire team (Clerical, Nursing, MRTs, TAs and physicians) work together and collaboratively with the ED Team, Inpatient units and the Odette Cancer Centre to ensure all patients receive diagnostic procedures in a timely manner in order to improve patient outcomes and experience.
Meal Time Matters Team, 6 & 7E, Holland Bone & Joint Program

With every interaction, this team exemplifies an approach to patient care which is consistent with Sunnybrook’s values and mission to help the organization achieve our strategic plan. We have observed that they demonstrate a unique, person-centered approach to care through their assessments and patient/family engagement.
The team is always supportive with providing preceptorship and championing high quality learning experiences to achieve noticeable growth in students. They have shown a commitment to student education and usher future colleagues into their roles as clinicians. Students have shared that the experience created for them has helped to give a sense of continuum of care and exposure to the different ways clinicians are able to practice in a hospital environment.
Their hard work, leadership and encouragement contribute to creating a safe working environment where staff can develop confidence when managing malnutrition in the elderly, completing calorie counts, implementing safe swallowing strategies and the recognizing the importance of supplemental nutrition.
Musculoskeletal and Complex Medical Rehab Interprofessional Team, A4, St. John’s Rehab Program

The interprofessional team on A4 plays a vital role in delivering high-quality inpatient rehabilitation to patients with musculoskeletal and complex medical care needs. Guided by Sunnybrook’s mission and strategic priorities, the team works in close partnership with patients and families to support individualized, patient-centered rehabilitation goals. They consistently embody the organization’s core values—demonstrating excellence through evidence-informed care, fostering collaboration across disciplines and with caregivers, and maintaining accountability in all aspects of their practice.
The rehabilitation team on A4 actively demonstrates diversity and equity by delivering patient-centered care that respects the unique cultural backgrounds and values of each patient and their family. For example, the team regularly accesses culturally appropriate resources to ensure care and services are relevant both during the hospital stay and at discharge. They also arrange for on-site interpreters to support clear communication when engaging patients and families in discussions about their rehabilitation goals.
Parking & Transportation Services Team

The Parking & Transportation Services (P&T) Team goes above and beyond to manage the traffic flow in our campus, ensuring patients are parking on the nearest available parking area to attend the clinic and providing patients directional instructions when necessary. The team is also actively focusing on collecting the s-taxis and docking these to the appropriate stations throughout the campus to facilitate the patient’s internal transportation from their vehicles to the physician office.
The team offers various support to the organization including:
- Managing approximately 10,000 vehicles transiting Sunnybrook on weekdays across the Bayview campus
- Over 50 parking gates and intercoms are handled by the P&T team
- Over 30 parking pay machines that are assisted by the team, providing customer support or troubleshooting when necessary
Prosthetics Team, Sunnybrook Centre for Independent Living

The Sunnybrook Centre for Independent Living (SCIL) Prosthetics Team is composed of skilled prosthetists and prosthetic technicians who collaborate to design, fabricate, and deliver customized prosthetic devices for individuals living with limb loss. Patients come from diverse backgrounds and present with a wide range of conditions—including diabetes, cancer, and trauma—which makes each prosthetic treatment plan highly individualized.
The team is deeply committed to fostering a culture of diversity, equity, and inclusion—not just in principle, but in practice. Each person is evaluated and supported through an individualized care plan developed collaboratively by prosthetists and prosthetic technicians who prioritize active listening, observation, and empathy. The team intentionally avoids assumptions or generalizations about what a patient might need. Instead, they tailor prosthetic solutions to align with the person’s lifestyle, values, and goals—whether that means designing a device optimized for basic transfers, creating an easy-to-don solution for someone with limited dexterity or visual impairment, or crafting a prosthesis suitable for high-performance sports or recreation.
Respiratory Therapy - Adult Team, Critical Care, Tory Trauma Program

At Sunnybrook, where the most complex and urgent cases are treated every day, Respiratory Therapists (RTs) stand as essential members of the interdisciplinary team. They bring advanced expertise, dedication to patient care and remain calm under intense pressure. RTs make an impact in every area of the hospital. In the trauma bay, they are first responders to patients arriving after severe motor vehicle collisions.
In the ICU, they oversee advanced ventilatory and oxygenation support for patients recovering from complex surgeries. When advanced imaging is necessary, RTs accompany ventilated patients to guarantee safe transport and monitoring. RTs also play a vital role in the journey toward recovery. They are central to ventilator weaning, extubation, and providing additional non-invasive support that patients may need during those critical moments of respiratory transition.
Whether called to a Code Blue or a Rapid Response, RTs respond with expertise and professionalism, bringing life-saving intervention to patients in distress. Their role also extends to some of the most compassionate work in health care, supporting patients and families through end-of-life care, including the withdrawal of life support and organ donation.
