Announcement regarding after-hours care at the Sunnybrook Academic Family Health Team (SAFHT)

In February of 2021, the SAFHT started a pilot project to provide better and broader access to patient care. We extended our clinic hours to include regularly scheduled evening clinic appointments Monday through Thursday from 5:00 to 8:00 p.m. as well as one weekend clinic each week.

After piloting the program for several months and reviewing feedback from patients, we have decided to extend our weekend phone hours to Saturdays AND Sundays from 9:00 a.m. to 3:00 p.m. There will continue to be one weekend clinic, usually on Saturdays but occasionally on Sundays. Bookings for the weekend clinic can be made the Friday before or the day of the clinic and will be based on availability. On long weekends, we will continue to run an additional clinic on the statutory holiday. Same day bookings can be made (based on availability) by calling your doctor’s office and following the voicemail instructions.

For urgent clinical issues between the hours of 4:30 p.m. and 8:00 p.m. Monday through Thursday and between 9:00 a.m. and 3:00 p.m. on weekends and holidays, please call your family doctors’ office and the voicemail will prompt you how to reach the after-hours physician. On Mondays through Thursdays after 8:00 p.m., Fridays after 4:30 p.m. and weekends after 3:00 p.m., please contact Health Care connect at 811 where a nurse will be able to provide advice on urgent matters.

After-hours clinics may be virtual, in-person or a combination, depending on the circumstance.
