Attention patients of Dr. Rezmovitz

Update - September 12, 2019:

The search is ongoing for Dr. Rezmovitz’s replacement.

Those patients who still wish to transfer to Dr. Rezmovitz’s new location are asked to please contact our office as soon as possible. As of early September, a mass data transfer is pending which would allow a more fulsome transfer of information for those patients who are moving. This will happen only once. Any future transfer will still allow for data exchange but the information will need to be inputted manually and will be in a much more abridged form. The cost for a records transfer remains $40 per patient.

In an effort to keep everyone informed, please note the following:

Dr. Jeremy Rezmovitz has left the Sunnybrook Academic Family Health Team as of June 30, 2019.

Due to circumstances beyond our control, we were unable to recruit to fill this position in time to transition directly to the new physician.

In the interim, coverage will be provided by the South Team Physicians and our Nurse Practitioner, Andrea Payne, until October 21, 2019. Dr. Samyuktha Adiga will be providing locum coverage starting October 2019. She will provide coverage until the end of January 2020, at which time we hope the permanent physician will be in place.

Please continue to book your appointments with your coordinator, Nicky, at 416-480-4934. Both Nicky (coordinator) and Liza (the team nurse) remain a constant for all patients. We are working hard to transition things as seamlessly as possible.
