North East Toronto Health Link Patients’ Advisory Council wins a 20 Faces of Change Award

The Patients' Advisory Council of the North East Toronto Health Link won the 20 Faces of Change Award, provided by the Change Foundation, an independent health care think tank in Ontario. The awards, handed out in Toronto on March 9, recognize those who have inspired positive patient-centered change in the health care system.

The Patients' Advisory Council, co-chaired by Lisa Priest, Director of the North East Toronto Health Link, and Bruno Geremia, a caregiver, co-designed a coordinated care plan for the Toronto Central LHIN which helped inform the provincial care plan, insisted on transparency of information and created a "All About Me" card to help reduce the number of times patients have to tell their stories to providers.

A care team based in Sunnybrook Hospital's emergency department conducts coordinated care planning on Health Links patients identified in real-time through an innovative information management system called Better Care, which was created by Sunnybrook's Information Management team for the Health Link.

Patients are enrolled into the Health Link and engaged in their care planning by working closely with clinicians. The Health Links's next phase will allow community providers of the Health Link to formally enroll their patients so they can be notified when those patients arrive in the emergency or are admitted to hospital.
