Office of the Resident and Family Experience
There may be times when you need help finding information or resolving a concern. The first step is to speak with the resident's nurse or to the person on the unit that can best address your concern (nursing team leader, social worker, patient care manager or physician).
If you require more information or further assistance, we encourage you to also contact the Office of the Resident and Family Experience. Our goal is improve the level of service and satisfaction that residents and their families have at the Centre. The ORFE will ensure that your perspectives and concerns are addressed in a safe, confidential setting and timely manner. Your feedback and ideas are valued and help improve the quality of care and programming that is provided.
How to contact the Office of the Resident and Family Experience
Whether in writing, by telephone or when leaving a voice mail on our confidential telephone lines, please be prepared to share the following information:
- Your name and contact information, including an email address
- If you are not the resident, please provide the resident's name and your relationship to him/her
- Details of your concern:
- The unit or location where the concern arose
- The names of those involved (if known)
- The date and approximate time when the situation occurred
- What has been done so far to try and resolve your concern(s)

If you have a concern on the weekend, during the daytime, please speak with your unit's Team Leader.
For weekends, during after hours and concerns that cannot wait until regular business hours please call Hospital Locating at 416-480-4244 and request to speak with the Shift Manager.
