Regional Stroke Best Practice Team

Clinic Scope

The Regional Stroke Best Practice Team is a specialized inter-professional team comprised of an occupational therapist, physiotherapist, speech-language pathologist and a social worker. As part of the North and East GTA Stroke Network, the team provides consultative services to persons recovering from stroke and their families, focusing on education and linking them with appropriate community resources. The Team also supports the implementation and sustainability of the North and East GTA Stroke Network initiatives and aims to provide leadership in stroke care throughout the continuum of care.

Eligibility Criteria

1. A person with a confirmed stroke or a person under investigation for a TIA

2. Referred from the ED, TIA/SPC clinics, and NVU neurologist at Sunnybrook Health Sciences Centre

3. A consultative assessment with the inter-professional team (OT,PT,SW,S-LP) for a person that may benefit from stroke prevention education, community reintegration and/or connection with stroke pathway

4. The Stroke Best Practice Team does not provide ongoing treatment

Exclusion Criteria

1. Patients not referred from the ED, TIA/SPC clinics, and NVU neurologist at Sunnybrook Health Sciences Centre.

2. Patient requiring ongoing treatment

3. No confirmed stroke and person is not under investigation for a TIA
