Making an appointment
- New Patients
To become a dermatology patient we require a referral letter from your family doctor or referring physician to make an appointment. This referral should include the patient's legal name, date of birth, home address, phone number(s), OHIP number and physician's signature. The referral must be faxed to the clinic of choice. Our fax number is 416-480-6897.
- Existing Patients
Patients who have seen our dermatologists in the last year may see their dermatologist for a follow-up appointment. If it has been more than 2 years since your last visit or there is a new problem, we will require a new referral from your family doctor or referring physician. Our main clinic number is 416-480-4908. Our email is dermatologybooking@sunnybrook.ca.
- Cosmetic Patients
For patients interested in cosmetic procedures, no referral is required. Call our Cosmetic line at 416-480-5222 or email us at cosmeticdermatology@sunnybrook.ca. A consultation fee may be charged.
