Join the Primary Care Wait List – For North Toronto Residents

Don’t have a family doctor or nurse practitioner?
If you live within our catchment (bordered by St. Clair, Bathurst, Sheppard and Victoria Park), you can join our wait list to get connected with a primary care provider. We accept patients living in this catchment area, with priority focus on M4P/M4S postal codes. We will also attach prenatal patients and their families in North Toronto who are seeing us for pregnancy care.

How to join:
Simply fill out the form below to register. Please note, submitting the form does not guarantee a provider at this time, but it ensures you’ll be considered as we expand our services and accept new patients.

What to expect next:
A team member will reach out to you within approximately 8 weeks with updates and next steps. We thank you for your patience as we work to improve health care access in your community.
