Neurology Post Hospital Discharge Clinic

Clinic Scope

Patients with neurological concerns that require assessment following a Sunnybrook inpatient hospital admission from Sunnybrook.

Eligibility Criteria

Catchment: Sunnybrook hospital internal referrals only.

Age: 18 and over

Diagnosis: neurological concern requiring assessment following a Sunnybrook inpatient hospital admission.

*Only internal referrals to Sunnybrook are accepted.

Exclusion Criteria

1. Patients without a suspected neurological diagnosis (e.g. vertigo suspected to be due to BP PV/peripheral etiology). Recommend referral to vestibular therapy and/or Dizzy Clinic

2. Patient suspected to have a neuromuscular disorder and has follow up arranged in the Neuromuscular clinic at time of discharge.

3. Patient is known to an outpatient Neurologist who actively follows them for their neurological symptoms/disorder (e.g. known diagnosis of MS followed by MS Clinic presenting with new flare, epilepsy presenting with breakthrough seizure(s).

4. Patients discharged from Sunnybrook from acute care admission - patients with planned repatriation to home hospital from Sunnybrook to have Neurology follow up arranged at home hospital
