Referring a patient

To refer a patient to our Division, please fax a referral letter addressed to the individual neurosurgeon, including the patient's legal name, date of birth, home address, phone number(s), e-mail address, OHIP number and physician's signature.

All referrals must be accompanied with reports of ALL relevant investigations (e.g. MR, CT, X-Ray or EMG results).

The referral must be faxed to the neurosurgeon of choice. Our general departmental fax number is 416-480-6085.
