Geriatric Psychiatry
Fellowship Description
The Fellow will have the opportunity to design a program to meet his/her needs and career goals whether the focus will be on research, education or clinical services.
Clinical Responsibilities: Supervision is available in a variety of clinical settings including a community team, outpatient department, inpatient service and consultation to an acute general hospital or chronic, long- term care units. The percentage of time spent dedicated to clinical services is completely negotiable.
Research: There are several major research themes currently underway at Sunnybrook Health Sciences Centre. These include geriatric psychopharmacology (with a focus on dementia and mood disorders), geriatric neuropsychiatry (e.g. traumatic brain injury, post-stroke depression), neuroimaging studies, bipolar disorders, driving and geriatric mental health issues, caregiver research, clock-drawing and cognitive screening. Whether the focus of the Fellowship is research activities or not, the expectation is that the Fellow will be involved in some type of research project.

Mood Disorders
Fellowship Description
This fellowship position is a full-time, one or two year position. The fellow will be expected to participate in clinical work, research and educational activities in the Mood Disorders Program at Sunnybrook Health Sciences Centre. Approximately 50 percent of the time, the fellow will be engaged in clinical activities which involve consultation for treatment resistant or newly diagnosed mood disorder outpatients and follow-up for a small number of complex patients. Approximately 40 percent of the time, the fellow will be engaged in research activities which will involve the development of a new and original research proposal, analyzing data from their own projects or from the clinical database, writing publications and presenting at scientific meetings. The clinic is currently involved in research in mood disorders that involves the epidemiology, phenomenology and biology of these illnesses as well as randomized controlled trials of new or novel mood stabilizing agents. We have a particular interest in studies of bipolar disorder and suicide. Fellows may choose research projects related to these areas specifically or other topics relevant to mood disorders. Educational activities will involve teaching of undergraduates, residents in Psychiatry and community physicians. In addition, the fellow will be encouraged to participate in advanced education, for example, at the Institute for Medical Sciences.
Educational Objectives
By the end of the program, the fellow should:
- Understand the principles of managing complex clinical situations in mood disorder patients
- Have developed an independent and fundable research protocol
- Learn the critical issues in current research of mood disorders

Neuropsychiatry
Fellowship Description
Research
A 2-year research fellowship is offered in two areas: the Neuropsychiatry of Multiple Sclerosis and the Neuropsychiatry of Traumatic Brain Injury (TBI). The Neuropsychiatry Program at Sunnybrook Health Sciences Centre is actively involved in research in these two areas, and the Fellowship offers training in clinical assessment and neuroradiological (MRI, fMRI) and neuropsychological assessment of patients with MS and TBI. The fellowship will provide training in MRI, fMRI and SPECT neuroimaging pertaining to research in the areas of MS and TBI.
Clinical
Approximately 25% of the time will be devoted to clinical neuropsychiatry, where the Fellow will be exposed to a wide array of neuropsychiatric disorders including seizure disorders, movement disorders, and the full spectrum of the somatoform disorders.
Expectations
The completion of the research project with publication of the results in peer reviewed journals and presentation of findings at scientific meetings.

A close liaison with colleagues in Behavioural Neurology at Sunnybrook Health Sciences Centre and the Baycrest Centre.

Geriatric Neuropsychiatry
Fellowship Description
This fellowship position is a full-time, one or two-year position. The fellowship will incorporate research,
clinical work and education, and while the proportion of time spent on these is negotiable, it is expected that fellows will be involved in a research project.
Research
The program is currently involved in research in motor vehicle collisions in the elderly (neuropsychiatric predictors and sequelae). Current research foci are: the risk of car crashes associated with dementia, depression, and medications, interventions to promote driving cessation in dementia, and various knowledge-to-action initiatives. There will be opportunity to collaborate with colleagues with expertise in neuropharmacology, neuroimaging, epidemiology, psychology, behavioral neurology, and geriatrics. If the candidate is interested in a research-based fellowship, they will be engaged in the development of a new and original research proposal, the analysis of data from their own projects or from the clinical database, writing publications and presenting at scientific meetings. These fellows will be encouraged to apply for operational funding for original research. For candidates with a primary clinical interest, opportunities will be available to create a research project by analyzing data from ongoing studies.
Clinical and Teaching
A wide array of clinical opportunities is available, incorporating consultation and follow-up in outpatient, inpatient, and chronic care settings. The clinical focus will be on the treatment of older patients with psychiatric morbidity related to neurological disease. Opportunities will be available to supervise residents training in geriatric psychiatry and medical students doing their core psychiatry rotations.
Educational Objectives
By the end of the program, the fellow should:
- Learn about the complexities of research design in geriatric neuropsychiatric illness.
- Present findings of original research, and submit this research for publication.
- Gain expertise in the consultation and longitudinal management of older patients with neuropsychiatric illness.

Cognitive Behaviour Therapy
Fellowship Description
Clinical Responsibilities
The Fellowship participant will attend a weekly cognitive therapy assessment clinic in which new patient referrals are evaluated regarding their suitability for CBT, and formulated within a cognitive model. The Fellow will also be expected to provide individual CBT treatment to a number of suitable patients under weekly supervision from Dr. Zaretsky. Opportunities for training in group CBT also exist.
Research
The Fellowship position has a number of opportunities for research participation depending on the interest of the participant.
Education
Fellowship participants will be expected to participate in weekly CBT seminars at Sunnybrook Hospital. A specialized course of supplementary reading on CBT will also be integrated into the Fellowship training experience. Fellows who have sufficient experience may also have the opportunity to provide supervision themselves and receive training in CBT supervision.

Psychosocial Oncology
Fellowship Description
This fellowship provides the supervised opportunity to become comfortable and proficient in assessing and managing psychiatric and psychological issues in the context of the diagnosis and treatment of cancer. It also includes developing expertise in grief, existential psychotherapy and individual and family support for those living with chronic medical illness as well as those who are dying. There is opportunity to participate in academic rounds clinical seminars as well as research.

Mood and Anxiety Disorders in Women Across Reproductive Transitions
Fellowship Description
The Women’s Mood and Anxiety Disorders Clinic: Reproductive Transitions is part of the Mood & Anxiety Disorders Program at Sunnybrook and staff work closely with the DAN Women & Babies program of the hospital. The aim of the fellowship is to enhance clinical experience with women suffering from mood or anxiety disorders across the reproductive life cycle. Assessment and treatment of perinatal mood and anxiety disorders is currently the focus of the clinic but exposure to other stages such as perimenopause is also an option. Both outpatient and inpatient experiences are available, including shared care. Consultation, assessment and short-term follow-up are provided. Our staff have expertise in both psychotropic medication and psychotherapies as potential treatment options. A number of research studies are ongoing and opportunity exists for involvement in existing projects or the creation of a new project. Experiences will be catered to fellow’s interests and ongoing educational needs / goals. Both clinical and research fellowships can be negotiated including a combination. Where the fellowship is primarily clinical, an academic deliverable is an expectation of the fellowship program. Research fellows will be encouraged to participate in advanced education, for example, at the Institute for Medical Sciences but this is not a requirement; in such cases, applying for and or receiving competitive funding is an expectation.

Fellows will be expected to participate in weekly seminars at Sunnybrook Hospital integral in teaching and or supervising younger learners.
Educational Objectives
By the end of the program, the fellow should:
- Understand the principles of assessing and managing complex clinical situations in perinatal mood and anxiety disorders or other stages of reproductive transitions.
- Have developed an independent and fundable research protocol those in a research stream.
- Have developed an academic piece of work for those with a primarily clinical focus.
- Learn the critical issues in perinatal research or reproductive transitions in women affecting the quality and consistency of data we use to make treatment decisions.

OCD & Related Disorders
Fellowship Description
This fellowship will provide residents with the opportunity to gain expertise with obsessive compulsive disorder, as well as related OCD-spectrum disorders such as hoarding, trichotillomania, and body dysmorphic disorder. The fellow will develop skills pertinent to the assessment and treatment of outpatients with these conditions, covering provision of consultations to community physicians, and hands on delivery of treatment to a selected cohort of appropriate outpatients. Expertise will be developed in delivery of both pharmacological and psychological treatments, focusing on specific CBT skills for this population. The fellow will also have the opportunity to co-lead group CBT for OCD and trichotillomania. Opportunity to assess the broader range of anxiety disorders may also be arranged.
There is significant opportunity to participate in research if interested, however this is not a requirement.

Collaborative Care
Fellowship Description
The Collaborative Care opportunities at Sunnybrook are diverse and flexible. I would be happy to work with you to develop a tailored experience to meet your educational/career related needs.
We provide support to our in-house academic Family Health Team (FHT). In addition, over the last years, due to our expanded videoconferencing capacity, we have developed relationships with various FHT across Ontario to provide collaborative care support to underserviced areas. These relationships focus on education and capacity building with less emphasis on direct consultation. Due to growing interest in this service, there will be opportunities to engage in program development and take on leadership roles.
Our program is expanding and we are happy to support fellow led initiatives. Also, for those interested in quality improvement projects, we are studying our model of collaborative care locally at Sunnybrook with the hopes of optimizing the program and developing metrics to evaluate outcomes.

Youth Bipolar Disorder
Fellowship Description
Research
The focus of this fellowship (1 year, with option of renewal) is on adolescent bipolar disorder. The selected fellow will be integrated into the overall activities of the Centre for Youth Bipolar Disorder, which includes post-doctoral fellows, pharmacology graduate students, undergraduate research students, and research staff. The Centre’s activities benefit from unique collaborations with medical imaging, medical biophysics, anesthesia, ophthalmology, and neurology. The fellow will gain exposure to current methods being applied in the laboratory, including neuroimaging (MRI), vascular imaging (ultrasound, peripheral arterial tonometry), computerized cognitive testing, serum biomarkers, genetics, and novel complementary therapeutics. Together with the supervisor, the fellow will identify a project, based on archival data, for which the fellow will have primary responsibility and first-authorship.
Clinical
Approximately 25% of the time will be devoted to diagnostic assessments, follow-up and treatment. The clinical focus is on adolescents with bipolar disorder as well as those at familial risk by virtue of having a first-degree relative with bipolar disorder. The fellow will be expected to work together with allied health professionals within the Centre, and to maintain high clinical standards with regard to evidence-based treatment, professionalism, and responsiveness.
Educational Objectives
- Develop expertise in the assessment and treatment of adolescents with, or at familial risk for, bipolar disorder.
- Gain experience in the process of scientific discovery and publication, and in the art and science of embedding research in care.
- Acquire skills working in a sub-specialized multidisciplinary clinical-research team.
