Youth Urgent Assessment Clinic

Sunnybrook’s Youth Urgent Assessment Clinic is a short-term consultation service available only upon referral from Sunnybrook’s Emergency Department when a youth has presented in psychiatric crisis.

This is a one-time urgent consultation service. Service is limited to support the immediate need. A maximum of three sessions to complete the consultation are available. After these sessions, the attending psychiatrist cannot guarantee follow-up care, however, recommendations for continued support may be provided.

Appointment times are limited. If a patient declines or cancels the appointment, it may not be possible to reschedule. Cancellation fees may also apply (see more details under Cancellations, below).

» How does it work?

  • Sunnybrook’s Emergency Department refers the patient to the Youth Urgent Assessment Clinic.
  • The patient will receive a phone call to arrange the appointment.
  • A follow-up email will be sent to the patient. It will include:
    • The appointment details.
    • Two questionnaires: one must be completed by the patient, the other is to be completed by the parent/guardian. Both must be sent back to the Youth Urgent Assessment Clinic [1-2] days before the appointment.
  • The parent/guardian must be available throughout the appointment because the doctor will ask for their involvement for a portion of the assessment.
  • This is a temporary, one-time consultation service and on-going care is not provided. The doctor will provide recommendations for ongoing care, as appropriate.
  • Appointment times are limited. To provide same-week urgent appointments for patients, if a patient declines or cancels the appointment, it may not be possible to reschedule. Cancellation fees may also apply (see more details under Cancellations, below).

» Cancellations

  • For cancellations made with less than 1 business day notice, please be advised of the following cancellation fees. Exceptions will only be made for extenuating circumstances.
    • $300 for a missed first appointment
    • $85 for a missed follow-up appointment

» Appointment checklist

  • All questionnaires (patient and parent/guardian): Complete and send to Youth Urgent Assessment Clinic 1-2 days before the appointment.
  • Patients must be in the province of Ontario during the appointment, as per OHIP guidelines.
  • The parent/guardian must be available throughout the appointment.
  • For late questionnaire submissions, please allow the doctor up to 15 minutes before joining the appointment to review the supporting documents.
