Interprofessional care at the Veterans Centre
Interprofessional care is the provision of comprehensive health services to residents by multiple health care professionals who work collaboratively with each other and with residents and families to deliver the best quality of care. It encompasses partnership, collaboration and teamwork to enhance care outcomes and is the cornerstone of our model of care. Interprofessional care also supports the overriding goal of assisting each resident in achieving his/her best possible life experience, through individualized and innovative approaches to care.
