Family Navigation Project co-hosts inaugural Canadian Health Navigation Conference
Sunnybrook’s Family Navigation Project (FNP) and NaviCare/SoinsNavi at the University of New Brunswick in Saint John are co-hosting the first ever Canadian Healthcare Navigation Conference which launches virtually on April 15-16, 2021.
The conference is about the advancement of patient navigation services across Canada. These services help connect patients and their families with resources by guiding them through various health, educational, and social systems.
FNP has long been a pioneer and leader in navigation after opening its doors in 2013 as the first program of its kind in Canada. Founded by Sunnybrook psychiatrist, Dr. Anthony Levitt, in partnership with patient families, it has been providing expert navigation of the mental health and addiction system serving teens and young adults ages 13 to 26 years old, and/or their families. FNP navigators help youth with complex needs find the most appropriate services that best match their needs.
“This conference is an incredible opportunity for novel collaborations among individuals in the healthcare navigation field including service providers, patients and families with lived experience,” says Dr. Levitt, Medical Director of FNP, and Chief of the Hurvitz Brain Sciences Program at Sunnybrook. “We are excited about the possibilities that may evolve from this inaugural conference aimed at strengthening the coordination of care for individuals with various needs find support and resources to move forward in their healthcare journey.”
