Note: these clinics are for Sunnybrook Academic Family Health Team patients.

For seniors 65 years of age and over and/or those with a chronic health condition:

• Wednesday, October 22 & Friday, October 24
  • 8:00 am — 12 noon
  • 1:30 pm — 3:30 pm

For healthy patients under 65 years of age:

• Thursday, October 30 & Tuesday, November 4
  • 5:00 pm — 7:00 pm

Please come to Room A124 with your Sunnybrook Card & your Ontario Health Card

Remember to wear a short-sleeved shirt!
