Help Build Your North Toronto Ontario Health Team

On December 4, 2019, Robin Martin, Parliamentary Assistant to the Minister of Health, announced the North Toronto Ontario Health Team will move forward with building a connected healthcare system, making it easier for patients and families to navigate the system and transition between providers. This is the result of an incredible amount of consultation with key stakeholders and collaboration among all members of the OHT. A roadmap has been developed to guide the work required to meet the OHT’s deliverables for the first year.

Six committees have been created with engagement and co-design with primary care, residents / patients / families / caregivers embedded in the process. Primary Care in North Toronto is currently led by Dr. Yoel Abells, Dr. Jocelyn Charles and Dr. Karen Fleming. Recognizing the importance of Primary Care, we are continuing to invite primary care providers to help co-design the OHT.
