Under the leadership of Dr. Jocelyn Charles, co-chair of the North East Toronto Health Link, and working in partnership with our partner providers, please find the latest version of our coordinated care plan. This care plan was co-designed by patients and caregivers who are members of the Health Links Patients' Advisory Council.
