MyChart

MyChart™ is Sunnybrook Health Sciences Centre’s online patient portal that streamlines the way health record information is accessed and shared. Sunnybrook recognizes that patients are key stakeholders in the delivery of care, and this service empowers them to manage their health by putting the necessary information in their hands.

The following is a series of Q&As to provide you with a better understanding of MyChart™.

1. What is MyChart™?

  • MyChart™ is a personal online health management and continuity of care solution designed specifically for Patients. MyChart™ provides access to medical record information, appointments, as well as several personal health records features to help manage your health journey in one place. MyChart users also have the ability to share access to their medical record information with family, caregivers, physicians, care teams (health professionals), pharmacists, etc.

2. Who has access to MyChart™?

  • Patients from participating partner organizations can register for a MyChart™ account and can also easily share information with family, caregivers, physicians, care teams (health professionals), pharmacists, etc.

3. What do patients have access to through MyChart™?

  • Medical Record Information - available 24/7 anywhere, anytime
    • Hospital Lab Results
    • Ontario Laboratories Information System (OLIS) Results
    • Appointments
    • Blood Bank
    • Radiology Images and Medical Imaging Reports*

4. How do patients begin using MyChart™?

Provide your email address at the time of registration and a MyChart PIN and registration instructions will be emailed to you. You can also submit an online enrollment request and a MyChart PIN will be emailed to you within 3-5 business days.

Other ways to obtain a MyChart PIN:

  • Contact MyChart™ Support at 1-833-221-2202 or support@mychart.ca
  • Visit Health Records Department locations during business hours:
    • Bayview – M1003 Health Records Express Desk & MyChart Registration
    • Holland Centre – Room 260

Once you have received your MyChart PIN, follow the instructions provided in the email to complete MyChart registration or visit www.mychart.ca and click on “Register” to complete your MyChart registration.

Please note: You will need your Health Card Number or hospital Medical Record Number to complete registration.

5. How long is the MyChart PIN valid for?

The MyChart PIN is valid for 10 days. If expired, the patient must contact MyChart support to get a new PIN.

6. Can I set up a MyChart account on behalf of a patient?

A designate MyChart account can be created for an individual who is permitted to take control of a patient MyChart Account on behalf of the patient as the patient’s PHIPA substitute decision maker (SDM) or Power of Attorney (POA) under s. 25 of PHIPA.

If you would like to obtain a MyChart PIN on behalf of a patient, please submit the following documentation to support@mychart.ca:

a. Completed designate form
b. Power of Attorney document
c. A copy of the patient's photo ID and photo ID of the individual requesting the designate account

7. Which browser is MyChart™ best compatible with?

MyChart is compatible with internet browsers such as Google Chrome, Firefox, or Safari.

    8. How does MyChart™ receive its information?

    MyChart receives clinical records directly from Sunnybrook’s health information systems and through self-entered information.

    9. Is MyChart™ secure?

    MyChart™ uses the same encryption as banks use for online banking. MyChart™ complies with applicable privacy laws.

    10. How to add records from another hospital (partner site) to your existing MyChart account?

    If you have registered a MyChart account with any of our participating hospital partner sites and have medical records with multiple MyChart hospital partner sites, your medical records will be displayed automatically when you log in to your MyChart account.

    Some MyChart hospital partners may require the user to complete a one-time request for medical records to be merged and displayed from other MyChart hospital partner sites.

    Please follow these steps if your medical records did not display automatically from your other MyChart partner hospitals:

    1. Login to your existing MyChart account
    2. Select 'Manage My Account'
    3. Select 'Manage Sites'
    4. Select 'Site'
      Note: For Michael Garron, William Osler and Sunnybrook only, you will need to enter your Medical Record Number
    5. Click on ‘Request Access’
      Note: Requesting access to North York General Hospital, Couchiching Family Health Team, Muskoka Algonquin Heath Care, Orillia Soldier’s Memorial Hospital, VHA Home Healthcare will navigate you to an external website to fill in an online enrolment application form

    Please note it can take 3-5 business days for the merge request to be processed.
