Clinics & Clinical Care

We work closely with our Veterans ProgramHurvitz Brain Sciences Program and the Regional Geriatric Program of Toronto to provide a full spectrum of patient care for seniors.

The acute geriatric unit provides multidisciplinary assessment and short-term treatment for seniors in all areas of the hospital. We provide specialized geriatric consultation, support to the geriatric emergency medicine nurse, ambulatory care visits - including our specialized falls program and wound care clinic - as well as providing in-home multidisciplinary assessment of the frailest homebound seniors through our geriatric outreach team.

In line with provincial directives, our participation in the multidisciplinary Impact Clinic for frail seniors has been enhanced, including the initiation of a telehealth project, reaching more seniors, province-wide.

Annually, we have more than 1000 clinic visits, 3400 day hospital visits, 520 home visits and 520 inpatient consultations.
