Inpatient Programming at St. John’s Rehab


What will my inpatient rehab stay look like?

Welcome & Admission
Shortly after your admission (within two days) your treatment team will meet and assess your needs.

Patient Care Plan
Your care team will work with you and your family to create a plan of care that focuses on your personal rehab goals.

Program Duration
Care teams meet to review each patient regularly. Discharge date will be provided within one week of admission.

Review of Progress
Your care team will work closely with you to identify any changes to your care needs and functional progress on a regular basis.

Discharge Planning
Your care team will review:
• Services you will need after you leave, equipment, medication management and community resources

Ready to Leave
Check out time is 9 a.m. The team will offer:
• Follow-up appointments, prescriptions and arrange/send referrals for continued services (if needed)

What will help my rehab journey?

Goals - The goal of rehab is IDEPENDENCE. The care team encourage you to practice daily tasks safely to achieve this goal. Be engaged - Actively participate in therapy sessions by asking questions, practicing exercises and staying motivated to achieve your goals. Inquiries / questions - Communicate with your care team. Ask questions, share concerns and stay informed about your progress. Be prepared for therapy - wear loose, comfortable clothing to allow free movement during exercises. Use supportive footwear for safety.

Home Passes 

As part of your rehabilitation, you may have the opportunity to spend a night or two at home. This can help ease your transition back to daily life, provided you have the necessary support available.

When the time is right, your care team will discuss this option with you and your support network. If you are approved for a home pass, please:

  • Provide at least three business days' notice to arrange the pass.
  • Check out with your nurse before leaving the hospital.

Essential Care Providers

Please appoint one caregiver as the contact person for any updates regarding the patient.

This will help the care team provide you with important information regarding stay, discharge and any other follow-up care in an efficient and timely manner.

Family Team Meetings 

If needed, patients and families will meet with the care team to discuss the patient’s progress in rehab, in addition to discharge and next steps. Family Team Meetings are at the discretion of the care team and are based on patient care and discharge needs.

Your care team will let you know if they think a Family Team Meeting would be beneficial.

Tips to Prevent Falls


Volunteer Meal Assistance Program

Trained volunteers can help at meal time to set up your trays, open packages and cut up your food. If you would like to participate in this program, please talk to a member of your care team.

Outpatient Services

St. John’s Rehab Outpatient Services offer assessment, treatment, and education to those with neurological injury.

Therapy may be offered on a 1:1 basis or in groups, and through telehealth when needed.

Therapy is tailored to meet each patient’s own needs and goals, including:

  • Caregiver support
  • Communication environment
  • Financial management
  • Health management
  • Life roles
  • Mobility
  • Social networking

Our Outpatient Services Team represents a range of rehabilitation professionals – physicians, nurses, physiotherapists, occupational therapists, social workers, speech-language pathologists and other dedicated professionals.

Ask your inpatient care team if a referral to Outpatient Services is appropriate once you leave the hospital.

Community Clinics - Patients may also benefit from our community clinics, including acupuncture, chiropody, chiropractic services, massage therapy and community exercise programs. These services are not covered by OHIP, but may be covered by extended health-care benefits.
