Sunnybrook Health Sciences Centre’s eHealth service called MyChart™ streamlines the way health record information is accessed and shared. Sunnybrook recognizes that patients are key stakeholders in the delivery of care, and this service empowers them to manage their health by putting the necessary information in their hands.

The following is a series of Q&As to provide you with a better understanding of MyChart™.

1. What is MyChart™?

  • My Chart is an online website also available through a mobile app (from the apple and google play store) where patients can create and manage their own personal health information based on clinical and personal information.

2. Who has access to MyChart™?

  • Participating hospitals’ patients have access to MyChart™, but they can electronically grant access to family caregivers, physicians, care teams (health professionals) pharmacists, etc.

3. How do patients access MyChart™?

  • MyChart™ is accessible from anywhere at any time through the Internet. Once registration is complete users can go to www.mychart.ca and log in to review their MyChart records.

4. What do patients have access to through MyChart™?

  • Personal and family health details (including allergies and current medications)
  • Online appointment requests
  • Online patient questionnaires
  • Clinic visit notes*
  • Radiology* (i.e. CT, MRI, X-Rays, Ultrasounds)
  • ECG
  • ECHO
  • Pathology*
  • Personal address book, compiled by the patients – physicians, caregivers, labs, clinics, etc.
  • Personal diary and personal tools
  • Labs* (i.e. Hematology, Microbiology, Blood Bank).
  • Ministry of Health Ontario Laboratories Information System (OLIS) – Lab Results
  • Patient Profile (for Sunnybrook Family Practice patients only)
  • Links to relevant disease-specific information and online events
  • Personalized health information (e.g. FAQs on procedures, etc.)

*Certain conditions / delays may apply.

5. How does MyChart™ receive its information?

  • Directly from health information systems and through self-entered information.

6. Is MyChart™ secured?

  • MyChart™ uses the same encryption as banks do for online banking. MyChart™ complies with applicable privacy laws.

7. How do patients begin using MyChart™ ?

Leave your email address at the time of registration or register online. A registration pin will be emailed to you.

  • Visit Health Records Department locations during business hours:
    • M1-003 Health Records Express Desk & MyChart Registration - Hours of Operation: 8:30 a.m. – 4:30 p.m.
    • Holland Centre – Room 260

Have your hospital medical record number (Sunnybrook Card) or health card number available.

Once you have your registration pin, log on to www.mychart.ca and click on 'Register' to complete your MyChart registration.

8. Who can give me my hospital Medical Record Number?

If you are unsure of your hospital medical record number, ask your physician/clinician.

9. How to add records from another hospital (partner site) to your existing MyChart account?

If you have registered a MyChart account with any of our participating hospital partner sites and have medical records with multiple MyChart hospital partner sites, your medical records will be displayed automatically when you log in to MyChart.

Some MyChart hospital partners may require the user to complete the following one-time request for medical records to be merged and displayed from other MyChart hospital partner sites.

Please follow these steps if your medical records did not display automatically from your other MyChart partner hospitals:

  • Login to your existing MyChart account
  • Select 'Manage my Account'
  • Select 'Manage Sites'
  • Select 'Site' (For Michael Garron, William Osler and Sunnybrook only enter the Medical Record Number)
  • Click on ‘Request Access’ (please note for North York General Hospital it will take you to an external site to fill in required information)

Please note it can take 3-5 business days for the request to be processed.
