Our COVID-19 Assessment Centre has moved
The Sunnybrook COVID-19 Assessment Centre is now located at 80 Armistice Drive. This building is located at the northern part of Sunnybrook's Bayview Campus, often called "The Estates."
Booking an appointment
As directed by the Ministry of Health, an appointment is required to receive a COVID-19 test at our Assessment Centre.

Who can receive testing at Sunnybrook’s COVID-19 Assessment Centre?
Sunnybrook's Assessment Centre provides COVID-19 testing to adults and children over the age of one if they meet any of following criteria, as per the Government of Ontario's guidelines. Please note: Appointments are required, as our centre no longer accepts walk-ins.
Adults:
- You are experiencing symptoms of COVID-19
- You were told by Public Health or notified by the COVID-19 Alert app that you were exposed to the virus (you may be asked to show Assessment Centre staff this notification).
- You have a close household contact who tested positive in the past 14 days.
- You work or live in a place where there’s an outbreak and you were told by Public Health or your Infection Prevention team to get a COVID-19 test.
- You work in a shelter, other congregate care setting or long-term care facility (or you must visit one and require testing to do so).

You were asked to get a COVID-19 test by the Ministry of Health or Public Health as part of other targeted group testing.
Note: Testing before international travel is no longer supported by Ontario’s assessment centres, as per government direction. Please find more information about international travel here.
Children (under the age of 16):
Please note: Toronto Public Health recently updated their guidance about who should get tested for COVID-19. A child can get a COVID-19 test at the Assessment Centre if they have any one of the following symptoms:
- fever or chills (temp of 37.8 or greater)
- cough (more than usual)
- difficulty breathing
- decrease or loss of smell or taste
- runny nose or stuffy nose
- sore throat or difficulty swallowing
- nausea, vomiting or diarrhea
- headache
- unusual or extreme tiredness or muscle aches
Sunnybrook is equipped to test children over the age of one for COVID-19. Any children under the age of one should go to their local Emergency Department for COVID-19 testing.
Note: Testing criteria for children may vary by city or region. Please check the guidance of the public health unit in your area.
Only people who meet any of the above criteria book an appointment to be tested at a COVID-19 Assessment Centre.

If you are having difficulty breathing or experiencing other severe symptoms, please call 911 or go to your closest Emergency Department.
Booking an appointment
Appointments for COVID-19 testing can be made using our online booking system.
Please note that:
- Appointments can be made up to 2 days in advance
- New time slots are available each day at 9:30 a.m.
- A maximum of 4 appointments can be made at the same time
- Interpreter services are available and can be arranged when you get here for your appointment
What to bring to your appointment
Please wear a mask to your appointment and bring your:
- Driver's licence or Ontario photo ID card (if you have one)
- Ontario Health Card (if you have one)
- Booking confirmation number (this will be sent to you via email and/or text message after your appointment has been confirmed)

Can't make your appointment? Please cancel to allow somebody else to book your spot
Appointments can be cancelled by following the link in your confirmation text or email. You can also follow the prompts on the online booking page. Only do this if you want to cancel all of the appointments attached to your booking. If you can no longer come to your appointment, please take a moment to cancel it. This helps our Assessment Centre and our community by opening up your testing appointment to another person who needs a COVID-19 test.

7 days a week: 9:00 a.m. - 7:30 p.m.
Appointments are required.
Note: Due to patient volumes and the need for physical distancing inside, you may need to wait outside of the assessment centre for a short time when you arrive. Please dress for the weather.
Getting here
People who are driving to the Assessment Centre: Please enter campus through the north entrance (Armistice Drive) and follow signs to the COVID-19 Assessment Centre
Please note the Assessment Centre is not accessible by TTC. If you need help getting to your COVID-19 test appointment, please call 416-480-4559. The phone line is monitored between 9 a.m. and 4 p.m. daily.
Accessibility
- Accessible parking is available close to entrance of Vaughan Estate
- The entrance is wheelchair accessible and has automated doors
- Accessible washroom available
- Assistance is available for clients who require accommodations due to a disability (e.g. line bypass upon arrival; swabs with special assistance in a more quiet area of the centre)

You must book an appointment for a COVID-19 test at Sunnybrook's COVID-19 Assessment Centre. No walk-ins.
Test results
If you have a valid (green) Ontario health card, you can access your test results using the Ontario Ministry of Health's online tool. Please retain your testing label to access your results if you do not have green Ontario health card.
